Spring Sports Begin Feb. 23rd! Important Info!
All athletes must complete and turn in the physical packet to the Athletic Office before they are eligible to participate in a sport.
The Physical Packet can be found under Info/Athletic Documents.
Click here for the athletic packet.
Please print and complete these forms (the physical form is included in this packet) and have your player turn them in to Vickie Conte in the Athletic Office. They will receive a PINK CARD that they will give to their coach. This clears them to participate in their sport.
Once you turn in your packet it is valid for one year. If you have turned in a physical packet this year for the Fall or Winter Season, it is on file. Students will come to the Athletic Office for a PINK CARD. PINK CARDS will be given out starting Monday, February 9th through February 20th. (the last day of finals) They can obtained in the Athletic Office before school from 7:00-7:45, at lunch from 11:40-12:25 and after school from 3:00-3:30.
Athletes will not be allowed to begin practice until they have turned in their complete physical packet and received their PINK CARD from the Athletic Office. We will not except incomplete packets. When the Coaches receive a PINK CARD from the athlete they will then be able to participate.
