1) Registration Forms due Feb 20th

What you will find in the Athletic Package for Lacrosse includes:
All forms due February 20, 2015. If you do not turn in your forms, you will not be allowed to practice or play Lacrosse.
- Parent Permission
- Physical Evaluation/Exam
- Injury option
- Emergency Information Form.
- Transfer Form
2) Regular Season Player Fee $200 Due by January 15, 2015
-You also need to pay this along with your registration papers. You may also submit a check no later then Feb 20 and hand it to Coach Brin or mail it to Brian Breitenwischer, 881 S. York Street, Denver, CO 80209

3) Varsity Tryouts 2015
Monday, February 23-Friday, February 27th.
*Tryouts are mandatory for those interested in playing Varsity.
4) Regular Practices for all levels will begin Monday, March 2, 2015.
5) Sign and Turn in a copy of your Emergency Card to the Coach or Jim Nachazel at the Kick-Off Dinner due by March 2, 2015.
A copy can be obtain from the Athletic Secretary.
6) Team Shorts Purchase Due by March 2nd
-Once you find which Team you are on, pay either by Check (pay at the Kick-Off) or online for the corresponding Team shorts due March 2 as follows:
- Varsity A $80. We will hold your check until we announce tryout results or pay online.
- Varsity 2 $40.
- Third Level $40.
7) Attend the Kick-Off Dinner March 2nd at the Rilko Center at 6pm
